HAS-BLED
Estimates 1-year risk of major bleeding in patients with atrial fibrillation receiving oral anticoagulation. Modifiable factors should be addressed rather than withholding anticoagulation in moderate–high risk patients.
Creatinine > 2.26 mg/dL (200 µmol/L), chronic dialysis, or transplant
Cirrhosis, bilirubin > 2× ULN, or AST/ALT/ALP > 3× ULN
Prior major bleed, anaemia, or severe thrombocytopenia
Time in therapeutic range < 60% (warfarin only)
Antiplatelet agent or NSAID
