SOFA

Quantifies organ dysfunction across six systems (respiratory, coagulation, liver, cardiovascular, CNS, renal) to monitor and prognosticate critically ill patients. An acute increase ≥ 2 points in the setting of suspected infection defines sepsis.

Respiratory — PaO₂/FiO₂

Coagulation — platelets ×10⁹/L

Liver — bilirubin (mg/dL)

Cardiovascular

CNS — Glasgow Coma Scale

Renal — creatinine (mg/dL) or urine output
