Mechanism

Capreomycin is an antimycobacterial cyclic peptide whose activity is similar to that of aminoglycosides.

Indications

Tuberculosis resistant to first-line drugs, in combination with other drugshard-to-treat nontuberculous mycobacterial infectionsdrug-resistant mycobacterium avium complexmycobacterium abscessusMDR-TB (as an alternative to aminoglycoside antibiotics, many streptomycin and amikacin resistant strains remain susceptible)

Dosing

Adult
BY DEEP INTRAMUSCULAR INJECTION: 1 g daily (max. per dose 20 mg/kg) for 2–4 months (or 60-120 days), then reduced to 1 g 2–3 times a week.
Renal adjustment
Reduce dose—consult product literature. Manufacturer advises caution if eGFR less than 30 mL/minute/1.73 m2.
Hepatic adjustment
Manufacturer advises caution in moderate impairment; avoid in severe impairment (no information available).
Max dose
20 mg/kg per day
