Mechanism

Dofetilide prolongs action potentials and the QT interval by potently blocking the IKr channel. Increased late Na+ current, likely due to inhibition of phosphoinositide 3-kinase, may also contribute. The drug has virtually no extracardiac pharmacological effects.

Indications

Maintaining sinus rhythm in patients with atrial fibrillationConversion of atrial fibrillation (AF) to sinus rhythmConversion of atrial flutter (AFl) to sinus rhythmMaintenance of sinus rhythm after conversion

Dosing

Adult
0.25–0.5 mg q12h
Renal adjustment
Decreases in dosage based on creatinine clearance are required to minimize the risk of torsades de pointes.
