Mechanism

Hetastarch is a complex mixture of ethoxylated amylopectin that, in a 6% solution, has colloidal properties approximating human albumin. It exerts oncotic pressure, expanding plasma volume slightly in excess of the infused volume, thereby retaining fluid in the vascular compartment.

Indications

Hypovolemia due to loss of plasma/blood (as substitute for plasma)Burns (acute phase only)Hypovolemic and endotoxin shockSevere trauma and extensive tissue damageTemporary measure in cases of whole blood lossImproving harvesting of granulocytes (by accelerating erythrocyte sedimentation)

Pharmacokinetics

Duration
Haemodynamic status improved for 24 hours or more
Half-life
17 days (for larger molecules)
Metabolism
Larger molecules are slowly broken down into smaller ones
Excretion
Smaller molecules (< 50,000 MW) excreted rapidly by kidney (40% of infused dose in urine in 24 hr)

Contraindications

  • Severe anaemia
  • Cardiac failure
  • Pulmonary edema
  • Liver disease
  • Renal insufficiency
