Mechanism

Idarubicin is an anthracycline antibiotic that intercalates with DNA, directly affecting transcription and replication. A more important mechanism involves forming a tripartite complex with topoisomerase II and DNA, thereby inhibiting topoisomerase II activity. These agents also possess the potential for generating free radicals.

Indications

Haematological malignanciesAcute leukemiasAcute myeloid leukemia (AML) (in combination with Ara-C)Acute lymphocytic leukemia (ALL)

Dosing

Adult
cyclophosphamide by the intravenous route at a high dose (e.g. more than 2 g) or in those who experienced urothelial toxicity when given cyclophosphamide previously. Anthracyclines and other cytotoxic antibiotics Drugs in this group are widely used. Many cytotoxic antibiotics act as radiomimetics and simultaneous use of radiotherapy should be avoided because it may markedly increased toxicity.…

Pharmacokinetics

Half-life
15 h (Idarubicin); 40 h (Idarubicinol, active metabolite)
