Mechanism

Liposomal amphotericin B targets ergosterol in fungal cell membranes, forming pores that lead to leakage of cellular contents and fungal cell death. The lipid encapsulation reduces toxicity and may improve penetration.

Indications

treatment of mucormycosismanagement of brain infection

Dosing

Adult
5–10 mg/kg once per day. Starting dosage is commonly 5 mg/kg per day. Dose escalation to 7.5 or 10 mg/kg per day for CNS mucormycosis may be considered. Doses of 5 mg/kg per day are probably adequate for nonbrain infections. No advantage to escalating the LAmB dose above 10 mg/kg per day.

Drug interactions

Deferasirox
Contraindicated
Textbook

Excess mortality, particularly in patients with active malignancy.

Deferasirox is therefore contraindicated as therapy in patients with active malignancy. Its role in patients who have diabetes mellitus without malignancy remains uncertain.
