Mechanism
Pancrelipase provides exogenous lipase, amylase, and proteases to replace deficient pancreatic enzyme production. This aids in the digestion of fats, proteins, and carbohydrates in the duodenum, preventing malabsorption.
Indications
Fat malabsorption (steatorrhea) and protein maldigestion due to pancreatic exocrine insufficiency (e.g., chronic pancreatitis, pancreatectomy, cystic fibrosis)Pain associated with chronic pancreatitis (evidence equivocal)
Dosing
- Adult
- Initial dose of lipase: 500 USP units/kg per meal, increasing up to 2500 USP units/kg per meal. Maximum recommended dose: generally not to exceed 2500 USP units/kg per meal or 10,000 USP unit/kg per day.
- Pediatric
- Children younger than 4 years have increased needs for lipase and initial doses are higher. Special dosing regimens for breastfeeding infants.
