Mechanism

Calcium chloride dissociates into calcium ions (Ca2+) and chloride ions. The calcium ions directly increase the serum calcium concentration, thus correcting hypocalcemia. This action is critical for restoring normal physiological functions dependent on calcium, such as nerve impulse transmission, muscle contraction, and blood coagulation.

Indications

Calcium deficiency statesDietary supplementSevere hypocalcemic tetanyLaryngospasmhypocalcemic tetanyextreme hyperkalemia (serum K+ >7 mEq/L)black widow spider envenomationmagnesium toxicity

Dosing

Adult
For severe hypocalcemic tetany and laryngospasm: given intravenously in a concentration of 10% (equivalent to 1.36 mEq Ca2+/mL). The rate of injection should be slow (not >1 mL/minute).

Contraindications

  • Hypocalcemia caused by renal insufficiency (undesirable due to acidifying salt properties)
  • Not used due to high irritation to gastric mucosa and tissues.
