Mechanism

Conivaptan is a nonselective V1a receptor/V2 receptor antagonist (aquaretic). It increases renal free water excretion with little or no change in electrolyte excretion by blocking V2 receptors in the collecting ducts, which normally increase water permeability. It also blocks V1a receptors. It does not affect Na+ reabsorption and thus does not stimulate the tubuloglomerular feedback mechanism.
