Mechanism

Ferrous gluconate is an oral iron salt used to treat iron deficiency anemia. Ferrous iron is more efficiently absorbed than ferric iron. Once absorbed, iron is incorporated into hemoglobin, which is essential for oxygen transport, thereby correcting anemia.

Indications

Iron deficiency anaemiaTreatment of iron deficiencyProphylaxis of iron deficiency (in malabsorption, menorrhagia, pregnancy, after subtotal or total gastrectomy, in haemodialysis patients, and in low birth-weight infants)

Dosing

Adult
Oral: For treatment of iron deficiency anemia, 200 mg elemental iron daily in 3 divided doses (alternatively, 3-4 tablets of 325 mg Ferrous gluconate [providing 36 mg elemental iron/tablet] daily). For prophylaxis, 30 mg elemental iron daily. Absorption is much better when taken on an empty stomach; however, side effects may be more prevalent.…
Pediatric
Oral: For infants and children, 3–5 mg/kg elemental iron daily in 3 divided doses.
Max dose
400 mg elemental iron daily

Pharmacokinetics

Bioavailability
About 25% of oral iron given as ferrous salt is absorbed.

Contraindications

  • Absence of a demonstrable iron-deficiency state
  • Presence of a serious underlying cause of anaemia that has not been excluded (e.g., gastric erosion, gastrointestinal cancer)
