Mechanism
Intravenous fat emulsions provide essential fatty acids (linoleic, linolenic) and a concentrated caloric source (9 kcal/g) for parenteral nutrition. The lipid particles (primarily soybean or olive oil triglycerides emulsified with egg phospholipid) are metabolized similarly to endogenous chylomicrons — they acquire apolipoprotein C-II in plasma, activating lipoprotein lipase on capillary endothelium, which hydrolyzes the triglycerides to fatty acids and glycerol for tissue uptake, energy production, and cell membrane synthesis.
