Mechanism

Low-molecular-weight heparins bind to antithrombin and accelerate the rate at which it inhibits various coagulation proteases, particularly factor Xa. At least half of the molecules are too short to provide bridging function and have no effect on the rate of thrombin inhibition by antithrombin.

Indications

maintenance treatment in patients with cancer-associated venous thromboembolismVTE

Dosing

Pediatric
May require monitoring
Renal adjustment
May require monitoring with an anti–factor Xa assay due to accumulation and bleeding risk. Contraindicated in patients with creatinine clearance below 30 mL/min.

Pharmacokinetics

Half-life
4-6 hours
Bioavailability
90% after subcutaneous injection
Excretion
Cleared almost exclusively by the kidneys

Contraindications

  • Creatinine clearance below 30 mL/min

Side effects

Common

Serious
  • Thrombocytopenia (<1% incidence)
  • Osteoporosis (lower risk than unfractionated heparin)
  • Hyperkalemia
