Prophylaxis: 2500 IU OD; Treatment of deep vein thrombosis: 100 U/Kg 12 hourly or 200 U/Kg 24 hourly

May require monitoring with an anti–factor Xa assay due to accumulation and bleeding risk. Contraindicated in patients with creatinine clearance below 30 mL/min.
