Mechanism

Caplacizumab is an engineered single-domain antibody fragment (nanobody) that targets von Willebrand factor (vWF). By binding to vWF, it inhibits the interaction between vWF and platelets. This action ultimately leads to the inhibition of platelet adhesion.

Indications

Acquired thrombotic thrombocytopenic purpura (specialist use only, in combination with plasma exchange and immunosuppressive therapy)

Dosing

Adult
Initially 10 mg for 1 dose by intravenous injection, given before plasma exchange, followed by 10 mg once daily by subcutaneous injection after each plasma exchange during, and for 30 days after finishing, daily plasma exchange therapy. Treatment may be continued after this if there is evidence of unresolved immunological disease.
