Mechanism
Digoxin immune Fab fragments bind to free digoxin or digitoxin in the blood, creating an inactive complex. This binding effectively removes the cardiac glycoside from its site of action on Na+,K+-ATPase, reversing the toxic effects. The Fab-digoxin complex is then renally excreted, enhancing the elimination of the drug.
Indications
Treatment of known or strongly suspected life-threatening digoxin toxicity associated with ventricular arrhythmias or bradyarrhythmias unresponsive to atropine and when measures beyond the withdrawal of digoxin and correction of any electrolyte abnormalities are considered necessaryLife-threatening digitoxin toxicitySerious arrhythmias due to cardiac glycoside intoxication
Dosing

- followed by careful control of the hypertension. She was initially treated with a diuretic (furosemide, 40 mg twice daily).
