Pharmacokinetics

Onset
plasma concentrations reach steady state approximately 8 weeks after thrice-weekly dosing
Half-life
3 to 5 days (in hemodialysis patients)
Excretion
cleared by renal excretion in patients with normal kidney function; hemodialysis is the predominant elimination pathway in patients receiving hemodialysis
