Mechanism

Golimumab is a monoclonal antibody. It acts by inhibiting the pro-inflammatory cytokine, tumor necrosis factor alpha (TNF-alpha).

Indications

Moderate-to-severe active ulcerative colitis (after inadequate response to conventional treatment options, or if conventional treatment is not tolerated or contraindicated)moderately to severely active rheumatoid arthritisactive psoriatic arthritisankylosing spondylitismoderately to severely active ulcerative colitismoderate-to-severe ulcerative colitis in adults not responsive or intolerant of traditional therapy

Dosing

Adult
corticosteroid dose cannot be reduced. Azathioprine p. 881 or mercaptopurine p. 963 [unlicensed indications] can be added to a corticosteroid or budesonide to induce remission. In patients who cannot tolerate azathioprine or mercaptopurine or in whom thiopurine methyltransferase (TPMT) activity is deficient, methotrexate p. 963 can be added to a corticosteroid.…

Serious
  • increased risk of serious infections
  • increased risk of lymphoma and other malignancies
  • infection (from immunosuppression, increased with steroids)
  • reactivation of tuberculosis
  • opportunistic infections with subsequent dissemination
  • increased incidence of non-Hodgkin lymphoma (causal role not established)
  • drug-induced lupus
  • multiple sclerosis–like syndrome
  • nonmelanoma skin cancer
