Mechanism

Protamine sulfate acts as a specific antidote to unfractionated heparin by forming a stable ion pair, neutralizing its anticoagulant effects. It only partially reverses the effects of low molecular weight heparins.

Dosing

Adult
Heparin reversal: 1 mg neutralises 80-100 units heparin (if given within 15 min of heparin). Max rate 5 mg/min. After heparin infusion: 25-50 mg IV once infusion stopped.
