Mechanism

Secukinumab is a human IgG1 monoclonal antibody that selectively binds to the cytokine interleukin-17A (IL-17A). By inhibiting IL-17A's interaction with its receptor, it blocks the release of pro-inflammatory cytokines and chemokines, thereby modulating inflammatory and immune responses.

Indications

Active ankylosing spondylitis (in patients who have not responded adequately to conventional therapy, NSAIDs, or TNF-alpha inhibitors)Active psoriatic arthritis (in combination with methotrexate or alone, in patients who have not responded adequately to DMARDs)Moderate to severe plaque psoriasisNon-radiographic axial spondyloarthritis (nr-axSpA)patients aged 6 years or older with moderate-to-severe plaque psoriasis who are candidates for systemic therapy or phototherapyadults with active psoriatic arthritisactive ankylosing spondylitisactive nonradiographic axial spondyloarthritis with objective signs of inflammation

Dosing

Adult
For Ankylosing spondylitis: 150 mg subcutaneously every week for 5 doses, then maintenance 150 mg every month. Review treatment if no response within 16 weeks. For Psoriatic arthritis: 150 mg subcutaneously every week for 5 doses, then maintenance 150 mg every month; dose may be increased to 300 mg according to clinical response.…
Max dose
300 mg monthly (maintenance for plaque psoriasis or for psoriatic arthritis requiring increased dose)
