Mechanism

Aminocaproic acid is an inhibitor of fibrinolysis. It prevents plasminogen from binding to fibrin, thereby inhibiting its activation to plasmin. This action helps stabilize fibrin clots and reduce bleeding by preventing the degradation of fibrin.

Indications

Oral bleedingreduce bleeding after prostatic surgeryreduce bleeding after tooth extractions in patients with hemophiliareduce intraoperative bleedingprevention or treatment of mucosal bleedingprophylaxis for dental proceduresadjunctive therapy with DDAVP or factor concentrate for dental extractions, tonsillectomies, and prostate proceduresBleeding in the gumsGastrointestinal tract bleedingBleeding during oral surgery

Dosing

Adult
clotbound. However, this regulatory system is overwhelmed by therapeutic doses of plasminogen activators. If the coagulation and fibrinolytic systems are pathologically activated, the hemostatic system may careen out of control, leading to generalized intravascular clotting and bleeding.…

Contraindications

  • ureteral obstruction by clots in patients with hematuria
  • upper urinary tract bleeding due to the risk of ureteral obstruction
  • Control of hematuria (due to concern for forming an occlusive clot in the lumen of genitourinary tract structures)

Increased risk of thrombosis or hypercoagulability, especially in patients with underlying thrombotic risk factors.

Use with caution. Monitor for signs and symptoms of thrombosis. Consider lower doses of one or both agents if co-administration is necessary.
