Mechanism
Clomifene citrate is an anti-oestrogen that induces gonadotrophin release by occupying oestrogen receptors in the hypothalamus, thereby interfering with feedback mechanisms. This action leads to an elevation of serum LH.
Indications
Female infertility due to ovulatory dysfunction
Dosing
- Adult
- Adult (female): 50 mg once daily by mouth for 5 days, initiated at any time if no recent uterine bleeding, or on or around the fifth day of cycle if progestogen-induced bleeding is planned or spontaneous uterine bleeding occurs. If ovulation does not occur, a second course of 100 mg once daily for 5 days may be given, at least 30 days after the first course.…

- 100 mg once daily for 5 days per course; long-term cyclical therapy not recommended beyond a total of 6 cycles.
Contraindications
- Abnormal uterine bleeding of undetermined cause
- Hormone-dependent tumours
- Ovarian cysts
- Long-term cyclical therapy beyond 6 cycles (due to possible increased risk of ovarian cancer)
