Mechanism

The Na+-Cl– symporter (called ENCCl or TSC) is expressed predominantly in the kidney and is localized to the apical membrane of distal convoluted tubule (DCT) epithelial cells. Bendroflumethiazide, as a thiazide diuretic, inhibits this Na+-Cl– symporter in the DCT. This inhibition prevents the reabsorption of sodium and chloride, leading to increased excretion of water, sodium, and chloride, thus promoting diuresis.

Dosing

Adult
Oedema: Initially 5–10 mg once daily or on alternate days in the morning; maintenance 5–10 mg 1–3 times a week. Hypertension: 2.5 mg daily in the morning, higher doses are rarely necessary.
