Mechanism

Benzathine benzylpenicillin, a beta-lactam antibiotic, exerts its bactericidal effect by interfering with bacterial cell wall synthesis. It irreversibly binds to and inactivates penicillin-binding proteins (PBPs), which are transpeptidases essential for cross-linking peptidoglycan strands in the bacterial cell wall. This inhibition prevents the formation of a stable cell wall, leading to increased osmotic pressure, cell lysis, and ultimately bacterial death.
