Mechanism

Cangrelor is a reversible inhibitor of the P2Y12 ADP receptor on the platelet surface. By blocking this key receptor, it attenuates platelet activation and recruitment to sites of vascular injury, thereby reducing thrombotic events.

Indications

Reduction of thrombotic cardiovascular events in patients with coronary artery disease undergoing percutaneous coronary intervention (PCI) who have not received treatment with oral clopidogrel, prasugrel or ticagrelor prior to the procedure and in whom oral therapy with these drugs is not suitable.reduction in the risk of periprocedural myocardial infarction, repeat coronary revascularization, and stent thrombosis in patients undergoing percutaneous coronary intervention who have not been treated with an oral P2Y12 inhibitor and are not given a glycoprotein IIb/IIIa antagonistpercutaneous coronary interventionPatients undergoing PCI following NSTE-ACS

Dosing

▶ BY MOUTH ▶ ▶ BY RECTUM Adult: 450–900 mg every 4 hours; maximum 3.6 g per day Acute migraine ▶ ▶ BY MOUTH Adult: 900 mg for 1 dose, to be taken as soon as migraine symptoms develop Prevention of pre-eclampsia in women at moderate or high risk ▶ ▶ BY MOUTH Adult: 75–150 mg once daily from 12 weeks gestation until the birth of the baby Mild to moderate pain (dose approved for use by community prac…

Pharmacokinetics

Onset
inhibits ADP-induced platelet aggregation within minutes
Peak effect
effect on platelet aggregation disappears within 1 h of discontinuation of the drug
Half-life
short half-life (rapidly dephosphorylated in the circulation to an inactive metabolite)

Side effects

Common
bleeding (greater risk than with clopidogrel during the coronary intervention)
Serious
  • Increased major bleeding events
