Mechanism

Eptifibatide is a glycoprotein IIb/IIIa inhibitor that acts as an anti-integrin drug. It exerts its antiplatelet effects by preventing the binding of fibrinogen and von Willebrand factor to the glycoprotein IIb/IIIa receptor on the surface of platelets. This inhibition prevents platelet aggregation, thereby reducing the risk of thrombotic events.

Indications

In combination with aspirin and unfractionated heparin for the prevention of early myocardial infarction in patients with unstable angina or non-ST-segment-elevation myocardial infarction and with last episode of chest pain within 24 hours (specialist use only)patients undergoing percutaneous coronary interventionmanagement of patients with non–ST-segment elevation acute coronary syndromeUnstable anginaCoronary angioplastyPatients undergoing PCI with a high coronary thrombus burden or thrombotic complications of PCI

Dosing

Adult
Initially 180 micrograms/kg by intravenous injection, then (by intravenous infusion) 2 micrograms/kg/minute for up to 72 hours (up to 96 hours if percutaneous coronary intervention during treatment)
Renal adjustment
Avoid if eGFR less than 30 mL/minute/1.73 m2. Reduce infusion to 1 microgram/kg/minute if eGFR 30–50 mL/minute/1.73 m2
Hepatic adjustment
Manufacturer advises caution; avoid in significant impairment (increased risk of bleeding complications, limited information available)

Pharmacokinetics

Peak effect
Platelet inhibition reverses within 6–10 hours
Half-life
10 to 15 min (plasma)
Excretion
cleared by the kidneys
