Mechanism

Nisoldipine is a calcium channel antagonist that produces its effects by binding to the 1 subunit of the L-type Ca2+ channels. This binding reduces Ca2+ flux through the channel, thereby altering the plateau phase of the cardiac action potential and reducing contraction in both cardiac and vascular smooth muscle cells.

Dosing

Adult
Steady-state serum concentrations of >0.25 mg/L itraconazole and >1 mg/L hydroxyitraconazole. <30 mL/min because hydroxypropyl-b-cyclodextrin excipient has reduced clearance with renal. Oral granules: 25 mg (56 packets), 50 mg (56 packets), 75 mg (56 packets). Tabs: 150 mg (56 tabs). 4e<6 mo and ≥5 kg (use oral granules): 25 mg PO Q12 hr
