Mechanism
Plerixafor is a bicyclam chemokine receptor antagonist that specifically inhibits the CXC chemokine receptor 4 (CXCR4). It acts by preventing stromal cell-derived factor-1α (SDF-1α) from binding to CXCR4. This blockade mobilizes CD34+ hematopoietic stem cells from the bone marrow into the peripheral blood.
Indications
Mobilise haematopoietic stem cells to peripheral blood for collection and subsequent autologous transplantation in patients with lymphoma or multiple myeloma (specialist use only)As an adjunct to G-CSF for patients with multiple myeloma or non-Hodgkin lymphoma who respond suboptimally to G-CSF alonemobilize hematopoietic stem cells (HSC) to the peripheral blood for collection and subsequent autologous transplantation in patients with non-Hodgkin’s lymphoma and MM (in combination with granulocyte colony-stimulating factor)
Dosing
- Adult
- Adults (body-weight up to 84 kg): 240 micrograms/kg daily, or 20 mg daily, by subcutaneous injection. Adults (body-weight 84 kg and above): 240 micrograms/kg daily by subcutaneous injection. Dosing is usually for 2-4 days (up to 7 days), administered 6-11 hours before apheresis, following 4 days of granulocyte-colony stimulating factor treatment.
