Mechanism

Sugammadex is a modified gamma cyclodextrin that acts by encapsulating the neuromuscular blocking agents rocuronium bromide or vecuronium bromide, thereby rapidly reversing their effects.

Dosing

Adult
Routine reversal of neuromuscular blockade induced by rocuronium or vecuronium: Initially 2–4 mg/kg by intravenous injection, then 4 mg/kg if required for recurrence of neuromuscular blockade. Immediate reversal of neuromuscular blockade induced by rocuronium: 16 mg/kg by intravenous injection.
Renal adjustment
Avoid if eGFR less than 30 mL/minute/1.73 m2.
Geriatric
Recovery may be delayed in the elderly.
