Mechanism

Corticosteroids, specifically glucocorticoids, exert broad anti-inflammatory efficacy by inhibiting the production of inflammatory cytokines. They do not directly relax airway smooth muscle but reduce bronchial hyperreactivity and the frequency of asthma exacerbations. Their action on airway obstruction is partly due to the contraction of engorged vessels in the bronchial mucosa and potentiation of beta-receptor agonists, but primarily through inhibiting the infiltration of asthmatic airways by lymphocytes, eosinophils, and mast cells.

Adjunct to aminosalicylates in acute mild to moderate ulcerative colitisAsthma

Dosing

Adult
For acute mild to moderate ulcerative colitis: 5 mg daily via oral modified-release tablet for a maximum of 4 weeks, taken in the morning. For asthma (inhalation, HFA) in adults (≥12 years): Low daily dose 88-176 µg, medium daily dose >176-352 µg, high daily dose >352 µg.
Pediatric
For asthma (inhalation, HFA) in children 5-11 years of age: Low daily dose 88-176 µg, medium daily dose >176-352 µg, high daily dose >352 µg. Not available for children 0-4 years of age.
Hepatic adjustment
Avoid in severe hepatic impairment (manufacturer advises, no information available).
Max dose
5 mg daily for acute mild to moderate ulcerative colitis.
