Mechanism

Fludrocortisone has very high mineralocorticoid activity and insignificant glucocorticoid activity. It increases plasma volume through its mineralocorticoid effects, which can be beneficial in conditions like postural hypotension.

Indications

Dosing

Adult
Neuropathic postural hypotension: 100–400 micrograms daily by mouth. Mineralocorticoid replacement in adrenocortical insufficiency: 50–300 micrograms once daily by mouth. Adrenocortical insufficiency resulting from septic shock: 50 micrograms daily by mouth (in combination with hydrocortisone).
Hepatic adjustment
Monitor patient closely in hepatic impairment.
Max dose
400 micrograms daily
