Mechanism

Fluvoxamine selectively inhibits serotonin reuptake by blocking the serotonin transporter (SERT), increasing synaptic 5-HT availability. It is unique among SSRIs in having significant sigma-1 receptor agonist activity, which may contribute additional anxiolytic and neuroprotective effects distinct from serotonergic mechanisms. Fluvoxamine is a potent inhibitor of CYP1A2 and CYP2C19 (and moderate inhibitor of CYP3A4), creating substantial drug interaction potential with theophylline, warfarin, and benzodiazepines.

Dosing

Adult
For depressive illness: Initially 50–100 mg daily, taken in the evening. Increase gradually up to 300 mg daily if necessary; doses over 150 mg daily are given in divided doses. Maintenance dose is 100 mg daily. For obsessive-compulsive disorder: Initially 50 mg daily, taken in the evening.…
